Agent 06 of nine · Referrals & Reputation · The referral source who sends the next one.
CareBravo®’s Referrals agent is AI referral and reputation software for Medicaid home and community based care agencies. The Referrals agent answers the referral the same day, shows you where each client came from, and asks the family for a review at the right moment. A person confirms anything that moves money or changes care.
The work we carry
Every new referral on one list, ranked by the clients you could lose, with the follow-up drafted for you.
The bravo you keep
The source who sends the next one.
We email you the highlights the moment you ask. No call. Nobody calls unless you ask.

Real product screens. Sample agency, sample data.

On the office screen: Where referrals come from, and what each source sends.
Why your current app can’t
A referral comes by fax or a case manager’s call. It sits in an inbox until someone has time. By then the family has called the next agency. Nobody knows which source sent last month’s clients, and nobody asked a happy family for a review.
What the Referrals agent makes from the tap
The Referrals agent answers the referral the same day, shows you where each client came from, and asks the family for a review at the right moment. Case managers see their own members without calling you. The source that sent one client sends the next one.
CareBravo is the Autonomous Care OS®: an operating system for Medicaid home and community based care, with an AI agent built into each of nine jobs. It starts as the EVV your state approved. Then it does the office work from the caregiver’s one tap at the door. How the whole thing works
Two parts of this job live in the product’s own distribution: case managers see their own members, and owners refer owners.
An owner’s exact words, answered on the screen, three ways to have it done.
Owner asks
The Referrals list is ranked by the clients you could lose, with the new referrals not yet called at the top.
One list.Cary drafts the follow-up for each source, including the day-14 check on a new referral, and the thank-you. You confirm.
Drafted by Cary, sent by you.Yes. Several home care systems now use AI for referral and reputation, mostly in scheduling and billing, and in most of them the AI suggests while your office does the work. In CareBravo, the Referrals agent does the work inside rules you write once: The Referrals agent answers the referral the same day, shows you where each client came from, and asks the family for a review at the right moment. It is one of nine agents, one for every job in the office, coordinated by Cary. A person confirms anything that moves money or changes care. CareBravo’s AI runs inside our HIPAA safeguards. Your clients’ records are not sent to public frontier AI models.
A case manager sees her own members only. She can read and request, and cannot change a schedule. Every view is logged, and you can see who looked at what.
When a referred agency pays its first invoice, you each get a $250 invoice credit. It is built into the product, and the details are on the refer-an-owner page.
Anything that goes out in your name waits for your confirm.
In Georgia’s CCSP program, clients choose their own agency. A nurse care coordinator offers the choice, may pass on a doctor’s recommendation, and uses rotation if the client has no preference. So referrals come from being enrolled, being easy to reach, and being known and trusted by coordinators and doctors. Read the full answer
Enrollment only lets you accept clients. It does not guarantee referrals. Coordinators look at the client’s choice, doctor advice, your county and hours, and whether you answered quickly last time. Check that your coverage area and contact details are current, then build relationships with the people who refer. Read the full answer
Rotation is how Georgia CCSP care coordinators choose an agency when a client has no preference. The care plan notes that rotation was used, and the coordination team assigns an agency. Clients can still choose their own agency when they enter the program, change agencies, or add a service. Read the full answer
No rule I found gives agencies a set number of minutes to answer. But Georgia coordinators work on short clocks: 24 hours to send a referral packet after services are brokered, and emergencies within 24 hours. Answer the same day, even if the answer is no. Read the full answer
Georgia care coordinators weigh the client’s choice, a doctor’s advice, special needs such as weekends or evenings, whether the service is available in the county, and whether the agency is enrolled. Families, in surveys, complain most about untrained caregivers, unannounced schedule changes and an office they cannot reach. Read the full answer
