Medicaid changes

Will my clients lose Medicaid in October 2026?

Some may, but it depends on the person and the state. On October 1, 2026, the federal definition of qualified immigrant for Medicaid narrows. Six-month renewals and work rules apply to expansion adults from December 31, 2026. We cannot say how many of your clients are affected. Ask your state Medicaid agency.

No one can say "your clients will lose Medicaid." Here is what the dates say.

October 1, 2026 (already here):

  1. The federal definition of qualified immigrant for Medicaid eligibility becomes narrower (OBBBA Sec. 71109). Clients or caregivers who are lawfully present immigrants could be affected, depending on their status and what their state does.
  2. Emergency Medicaid funding changes for states.

December 31, 2026 and after:

  1. Six-month renewals apply to adults in the expansion group.
  2. Work rules apply to adults 19 to 64 in the expansion group, with exemptions for medically frail and disabled people.
  3. Retroactive coverage shortens for applications on or after January 1, 2027.

For older adults and people with disabilities on long-term care waivers, these items often matter less than the state’s funding and the waiver’s slot limits. Waiting lists are already common: KFF counted over 710,000 people on HCBS waiver waiting or interest lists in 40 states in 2024.

What helps: ask clients to open mail from Medicaid, update their address, and call the number on any letter. For any single client, the state decides.
