Why was my EVV visit or claim rejected, and how do I fix it?

Most rejections come from a visit record that is missing, incomplete, or does not match the claim or authorization. In Georgia, the state EVV system checks for required data before a claim goes to GAMMIS, and rejected claims are fixed and resubmitted through that same system. If you are stuck, contact Netsmart support with names, dates and screenshots.

A rejection means the state system could not match your claim to a good visit record. Common reasons include a visit with no check-out, a visit that does not match the schedule, a client or authorization that is not in the system, or a claim for more units than the visit supports.

How Georgia handles it:

  1. All claims for EVV-required services go to GAMMIS through the State EVV solution.
  2. The State solution checks for required data before the claim goes.
  3. Rejected claims are corrected and resubmitted through the State solution.
  4. You can follow claim status in the GAMMIS portal, on your remittance advice, or on the EVV dashboard.

Steps to fix:

- Open the Claims Console and find the unmatched or rejected visit.

- Check times, caregiver, client and service code.

- Complete a missed clock-in or clock-out with a reason code if it is a real miss.

- Resubmit.

Still stuck? Contact Netsmart Customer Support at 1-833-483-5587 with the client names, dates of service, what you tried and screenshots. For missing authorizations or clients who are not listed, contact the DCH EVV Program team at evv.medicaid@dch.ga.gov.

Georgia’s pages do not list individual rejection reason codes, so use the training videos in the Admin Console.
