Who writes the care plan and who approves hours for CCSP and SOURCE clients in Georgia?
In CCSP and SOURCE, a care coordinator does the assessment, builds the care plan, and puts the hours on a Service Authorization Form. A doctor, or the case management agency’s medical director, approves the plan. Your agency does not write the plan. You deliver the hours that are authorized.
In Georgia’s Elderly and Disabled Waiver, the plan and the hours come from the client’s care coordinator, not from your agency. Here is the usual path from the state’s manual.
- The client is assessed by the Area Agency on Aging (or a SOURCE site for people with full Medicaid).
- Alliant Health checks nursing home level of care and issues a prior authorization that can last up to 365 days.
- The care coordinator facilitates the assessment and planning, writes the care plan, and authorizes services on it.
- A doctor, or the case management agency’s medical director, approves the plan of care.
- The client picks an agency. The client does not have to pick the agency that made the referral.
- The care coordinator sends the Service Authorization Form (SAF). Agencies do not submit it themselves.
- Your agency gets a packet with the level of care, assessments, and care plan.
What this means for you: you bill only what is authorized. If a client needs more hours, the request goes back through the care coordinator. Keep your visit notes clear, because the coordinator uses what you report.
