What do Medicaid denial codes mean on my claim?
A denial code is the payer’s short reason for not paying a line on your claim. Georgia Medicaid uses four-digit codes. For example, 2003 means the client was not eligible on that date, 3052 means authorized units are used up, and 4257 means a modifier is wrong. Look up the code, fix the cause, then resubmit.
Every claim comes back with a reason when it is not paid. Georgia Medicaid shows a four-digit code on the remittance. Federal remittances also use standard claim adjustment reason codes, but the Georgia codes are the ones to look up first.
A few you may see:
- 2003: the client was not eligible on one or more dates of service. Check eligibility for the whole span before you bill.
- 3052: the authorized units are used up. Check the authorization and ask for more units.
- 3011: the date of service falls outside the authorization dates.
- 3044: the rendering provider ID does not match the authorization.
- 4257: the modifier does not meet the billing rule. Fix it and resubmit.
- 5646, 5042, 5048: exact duplicate of a claim already in history. Find the paid claim.
Here is the key point. A denial is not always your mistake. Some are eligibility changes you could not see. Others are small data errors, such as a wrong modifier. Read the code, match it to your authorization and your visit data, and correct what is wrong.
The state’s list changes, so check the current GAMMIS denial guidance and your program manual before you rely on any one code.
