Referrals & Reputation

How do Medicaid residents find my personal care home? How do I fill open beds?

In the Elderly and Disabled Waiver, the client chooses the home. The care coordinator arranges services and sends the chosen home a packet. The client does not have to pick the home that referred them. Your home must be enrolled for Alternative Living Services first. DCH also lists licensed homes on its public facility search.

Waiver clients do not get "assigned" to a home. The state’s guide describes the path:

  1. The Area Agency on Aging assesses the person and refers the case to the waiver.
  2. The care coordinator arranges services. The client chooses a provider. The guide says a client "is not obligated to select the provider that may have made the referral."
  3. The chosen home receives a packet with level of care, assessments and the care plan.
  4. The care coordinator, not the home, sends the Service Authorization Form.

What you can control:

  1. Be enrolled for Alternative Living Services, with a full license and no restrictions.
  2. Be easy to reach. Care coordinators call homes when a client needs a place.
  3. Keep your inspection record clean. DCH inspection reports are public on its GaMap2Care search, so families and coordinators can look you up.
  4. Show up for the state’s information sessions and keep your provider details current. A Change of Information form goes through GAMMIS.

Before you offer or accept anything of value for a referral, ask DCH and your attorney what is allowed.
