Personal Care Homes

How do I get a personal care home license in Georgia, and what does it cost?

You apply to DCH for a permit. The fee rule lists a $300 application fee plus a license fee based on size: $350 under 25 beds, $750 for 25 to 50 beds, and $1,500 over 50. The application needs a property sketch, proof of ownership or a lease, and ownership details. Homes with 25 or more beds also need a financial affidavit.

Personal care homes are permitted by DCH’s Healthcare Facility Regulation Division under Chapter 111-8-62.

What Rule 111-8-62-.05 says the application needs:

  1. The application in the format DCH requires, complete with all attachments and fees.
  2. A sketch, plat, photos or drawing of the home and grounds, showing windows, doors, room sizes and bed placement.
  3. Proof of ownership or a lease.
  4. The administrator or on-site manager’s name (or give it before the permit is issued).
  5. Ownership details: officers, anyone owning 10 percent or more, and the registered agent.
  6. Proof you cleared local zoning with local officials.
  7. For homes with 25 or more beds at first licensing: a CPA’s affidavit that you can operate for two years.

Fees, from Rule 111-8-25:

  1. $300 application fee.
  2. License fee: $350 (under 25 beds), $750 (25 to 50), $1,500 (over 50).
  3. A 25 percent discount applies to homes with current approved accreditation.

No home may operate or admit residents without a current permit.

Fees and forms change. Use the "Facility Permit Applications" page on DCH’s site, or call the PCH team at 404-657-5700.
