How do I enroll as a Medicaid home care provider in [state]?

First get your state license and tax numbers. Then apply through your state Medicaid agency’s provider enrollment portal. If you want waiver clients, there is usually a second step with the program that runs the waiver. Search ’[state] Medicaid provider enrollment’ and read the waiver provider manual before you apply.

Most states follow the same pattern, but the details differ.

  1. Set up the business: entity, tax ID, and a National Provider Identifier (NPI).
  2. Get your state license for the services you will bill.
  3. Apply for Medicaid enrollment through the state’s portal. Expect to upload ownership details, insurance, and background check proof. Many states charge no fee for this step, but check yours.
  4. If you want waiver clients, find the waiver’s own provider rules. Some states add an information session, a limited open enrollment window, or a site visit.
  5. Complete any required training before you are given a provider number.
  6. Register for EVV and for your billing system.
  7. Revalidate when the state asks.

Example: Georgia uses the GAMMIS portal. For its Elderly and Disabled Waiver it also asks for an information session, a September enrollment window, and credentialing that can take up to 51 business days.

Where to find yours: the state Medicaid agency site, its provider manuals, and the waiver operating agency’s page. Ask for the current window dates in writing.
