How do I become a CCSP or SOURCE provider in Georgia?
Read Section 601 of the CCSP/SOURCE General Services manual on the Georgia Medicaid portal, hold a full license, attend a state information session, then apply through the GAMMIS Enrollment Wizard during the September window. The manual also asks for 12 months in the business and $1 million in liability insurance.
The 2026 EDWP General Services manual sets the path. In order:
- Hold a full license with no restrictions. Provisional licenses are not accepted.
- Have at least 12 consecutive months of experience in the business before you apply.
- Attend a state information session. It must be within six months before you apply. Older state guidance says sessions are held in February and August.
- Apply through the GAMMIS Enrollment Wizard with the Appendix CC notice and Appendix DD attachments. Open enrollment is in September for most services. March is for Structured Family Caregiving only.
- New applicants may apply for only one service, with a few exceptions (such as PSS/X and skilled nursing).
- Credentialing review can take up to 51 business days. A site visit may happen.
- Complete required new provider training before you are given a Medicaid provider number.
Other rules to plan for: $1 million liability coverage per occurrence and $3 million aggregate (plus workers’ compensation for private home care providers), fingerprint background checks, an office in Georgia open at least 8 hours a day Monday through Friday, and a Georgia-resident RN.
Dates and rules change. Confirm the next enrollment window with the Georgia Medicaid portal.
