Can my agency offer Structured Family Caregiving in Georgia?

Structured Family Caregiving is a service in Georgia’s CCSP and SOURCE waivers. A live-in family caregiver is supported by a health coach, and the agency is paid a per diem. Georgia takes new applications for it in March, a different window than other services. Check the current manual for the rules.

Structured Family Caregiving (SFC) lets a live-in, unpaid family caregiver get training and support so the person can stay at home. It is part of the Elderly and Disabled Waiver (CCSP and SOURCE).

What the state has said:

  1. The service gives the caregiver ongoing phone or electronic support from a health coach, plus a review of the caregiver’s daily notes.
  2. The 2019 public notice said the client needs a live-in unpaid family caregiver and an assessed need for at least five hours of personal support a day.
  3. It is paid at a per diem rate, which offsets personal support hours.

What the 2026 manual says about becoming an SFC agency:

  1. Open enrollment for SFC applicants is in March. Other services use September.
  2. You attend an information session within six months before you apply.
  3. SFC enrollment includes an onsite visit.
  4. The manual points SFC agencies to sections of the Personal Support Services manual.

The 2019 notice is old, and rules and rates change. Read the current EDWP manuals, or ask DCH, before you decide whether SFC fits your agency.
