Indiana EVV and Medicaid home care, in plain words.
Indiana uses Sandata to check Medicaid personal care and home health visits. You can use Sandata’s free tool or your own EVV app, but each visit must show as verified in Sandata before you bill.

Indiana at a glance
EVV, Medicaid and billing in Indiana.
Dates that matter: Personal care: claims suspended for 7 days from 28 Feb 2024, then denied for missing/unverified EVV from 1 Apr 2024 (EOB 0951/0952). Home health: post-and-pay in 2024, with final enforcement ’tentatively summer 2024’; confirm current status. PathWays for Aging launched 1 Jul 2024. 2026 waiver amendments effective 1 Aug 2026.
Licensing: Personal Services Agency (PSA) license from the Indiana Department of Health for non-medical in-home care; home health agencies have a separate IDOH license. Waiver agencies also need FSSA/OMPP HCBS certification, then IHCP enrollment, then MCE contracts for PathWays.
Managed care: Yes for 60+: PathWays for Aging managed care entities (MCEs) are Anthem, Humana and UnitedHealthcare; PathWays claims go to the member’s MCE. Health and Wellness and TBI waiver services are billed fee-for-service through IHCP.

Same rules, same tangle
In Indiana, the visit still gets typed four more times.
The caregiver taps in. The visit goes to Sandata. Then someone in your office carries it by hand into payroll, into billing, into the chart, and to the family on the phone. Every carry is a chance for the numbers to drift. When they do, Indiana denies the claim, and you find out weeks later. That is Tangleware™: good tools, and an owner in the middle.

CareBravo works with the EVV systems states use, including Sandata, HHAeXchange and Netsmart. What Indiana owners ask.
Will Indiana deny my personal care claim if EVV doesn’t match?
Yes. Since April 1, 2024, IHCP denies personal care claims that do not have a verified visit in the Sandata Aggregator. Before that date, claims were held for seven days to give agencies time to fix them. Check that each visit shows Verified in Sandata before you send the claim.
Can I use my own EVV app instead of Sandata?
Yes. Indiana offers Sandata’s tool for free, but you may use an alternate EVV system if it sends visits to the Sandata Aggregator. Register through FSSA’s Alternate EVV Provider Registration page. For help, contact inaltevv@sandata.com, or call IHCP at 800-457-4584, option 5.
Who do I bill for a client who is 60 or older?
Most Medicaid members 60 and older are in PathWays for Aging, which started July 1, 2024. Their waiver services are billed to their managed care entity, which is Anthem, Humana or UnitedHealthcare, not to IHCP. You must be contracted with that plan. Members under 60 on the Health and Wellness Waiver are billed through IHCP.
What license do I need to offer non-medical home care in Indiana?
Non-medical in-home care agencies need a Personal Services Agency license from the Indiana Department of Health. To serve Medicaid waiver clients, you also need FSSA’s HCBS certification and IHCP enrollment. To serve PathWays members, you also need a contract with each plan you will bill.
